Appointment Types

Aetna, United Health Care and Oxford 

I am an in-network provider for Aetna, United Health Care and Oxford, which allows me to submit on your behalf.  Reimbursement of services by your insurance company will be dependent upon whether you have met your deductible, whether it is a covered service under your plan, and whether you have a co-pay or there is a cap on number of visits.

All other insurance plans require self-pay at time of service appreciated.  I will provide you a completed insurance form (HFCAA 1500) with correct coding, which you can submit to your insurance provider for out-of-network reimbursement. 

I also accept all forms of payment: cash, check, and all major credit and debit cards.

When services start, you will be asked to sign an acknowledgment document of the following policy: 

If for some reason, you should need to reschedule/cancel your appointment, Back to Basics Nutrition Consulting, LLC appreciates 24-hour notification.  Of course, life happens and sometimes this is not possible. 

As result, the following will apply:

·  First cancellation not within the 24-hour window, no charge. 

·  Second cancellation not within the 24-hour window, 50% of appointment fee will be charged.

·  Third and beyond not within the 24-hour window, 100% of the appointment fee will be charged.

·  No show, 100% of the appointment fee will be charged.
